I don’t think those Republican Congressmen opposed to the Health Care Bill are afraid of punishment by the administration. Rather, they are hinting at a primary v Trump. Even at this early date they don’t think he could win in 2020 given the screw-ups thus far.
And a vote for would be unpopular in their districts where to many voters are dependent (hooked) on Obamacare and fear loss of coverage.
