• the url (copied from the address bar) where you discovered the issue
• what you thought was going to happen
• what actually happened
• a screen shot of what happened
• the browser you’re using (Chrome, FireFox, Safari, Internet Explorer, etc)
• the operating system you’re using (Windows, Mac, iOS, Android, etc — include version numbers)
