There is also Liberal Fascism: The Secret History of the American Left, From Mussolini to The Politics of Meaning by Jonah Goldberg.

It provides a good history of the rise of Progressivism from the early 20th century up through more modern times.  It might not cover the amount of depth on the subject you want but I found it insightful in how Progressivism was very much in vogue in the early 20th century.

Peter Meza: Thanks - I already read it, but obviously I need a re-read.  Parts of the book stuck with me, but exactly how Progressivism arose to the extent that it is covered did not.
